Here is what an Illinois Driver’s Abstract looks like. The example shown on this page is blank. If you obtain a printout of your own driver’s abstract from the Illinois Secretary of State, it will contain a list of the different traffic dispositions that have been entered on your driving record.
The “court purposes” version of this document will show prior convictions and any instances of court supervision. If you are suspended or revoked, any suspensions or revocations will appear as well.
Providing attorney Matt Hoffman a copy of your “court purposes driver’s abstract" will allow him to quickly “diagnose” any problems with your driver’s license. In the event he represents you in traffic court, your court purposes driver’s abstract also will enable him to determine what types of dispositions should be avoided on any traffic violations that have been charged against you.
The major columns on the front side of your Illinois driving record indicate the “type of action,” “date of arrest,” “effective date of action,” description of action,” “accident or ticket number,” “termination date of action,” and “stop in effect.”
The back side of your driver’s abstract contains a list of codes. These codes indicate the type of previous action that has been taken or recorded by the Illinois Secretary of State. There is a large number of codes. You can purchase a copy of your driver’s abstract at any Illinois Driver’s License Facility. The cost of an abstract is $12.00.

Here you will find a copy of a typical license suspension letter from the Office of the Illinois Secretary of State. This letter, titled "Notice/Order of Suspension," may be sent out just a few weeks or even days before a suspension is scheduled to start.
The Notice/Order of Suspension details when your driver's license suspension is scheduled to begin and end. It states the reason the suspension is being implemented. It also lists the record of different convictions, including where and when each happened, that have led to your suspension.
Ticket or case numbers will be included. These numbers identify the court case docket number of citation number that formed the basis of suspension action. The Notice/Order of Suspension letter also will state the jurisdiction in which any specific alleged traffic violation was adjudicated, and frequently will contain a telephone number to the relevant Clerk of the Circuit Court.
Important dates also will be provided, specifically, the alleged dates of the offenses and the dates on which convictions for those offenses occurred.
If you have recieved a Notice/Order of Suspension, that does not automatically mean that nothing can be done to prevent the suspension from taking effect. It may also be possible to end a suspension that has already begun.
